Why we exist

Commercial insurers underpay practices by 1 to 3 percent of net revenue on average, and deny a further share of claims for reasons that could have been fixed. That average is held down by hospital systems, which have entire teams watching for it. In an independent practice, where nobody is watching, the number runs much higher. When we audited thirteen months of claims at a hand and upper-extremity orthopedic practice in Miami, about 1,900 claims, over 20 percent of net insurance revenue had been paid short, downcoded or denied for fixable reasons.

Here's why the money survives. Every underpaid claim closes at a zero balance, so it looks paid. Every fixable denial sits in a work queue nobody has time for. Billing teams keep claims moving, which is their job, and nobody is staffed to go back and check finished business against contracted rates. We started Clearia to be the ones who look.

How we work

• Concierge, in person. We work at your practice, not from a call center. People read your remittances, and studying payer behavior is what we do all day.
• On the payer's side of the ledger only. We never touch claim submission, posting, or your billing workflow. Nothing we do is an audit of your team.
• Your name, our filing. You see every finding and set the scope once. Then we file as your authorized representative, under the practice's name, and you get a record of everything that goes out.
• By the book. An underpayment reconsideration is routine, contractual stuff. We use the dispute mechanisms your contracts already provide, professionally, and we keep the paper trail to match.

The incentive structure

The diagnostic is free. Our fee is a percentage of dollars actually recovered, and there is nothing upfront, ever. That means we carry the risk of being wrong, which is exactly why we won't take an engagement unless the diagnostic shows real recoverable dollars. Honestly, it wouldn't make sense to.
